The organizations listed below are official Connecting Kids to Coverage sub‑grantees. Each one has trained staff ready to help families enroll in or renew Medicaid and CHIP coverage. Support is available for children, parents, and expecting mothers to ensure they receive the health care benefits they deserve.

Access Community Health Network

Founded in 1991 to provide primary and preventive care services in targeted areas on Chicago’s South and West sides with high levels of poverty and poor health outcomes, ACCESS has grown into a diverse network of 35 health centers that serves more than 150,000 patients each year across Chicago and suburban Cook and DuPage counties.

As the nation’s sixth largest network of federally qualified health centers, ACCESS continues to innovate and evolve its care delivery model to address the total health needs of their patients. By taking a comprehensive, integrated approach to our medical practices, ACCESS is able to bring together the right services and resources at the right time to support our patient’s overall health and wellness.

AHS Family Health Center

AHS Family Health Center was established in 2001 as an independent nonprofit organization with 501(c)(3) status. Their mission is to improve access to primary care for underserved communities on Chicago’s north side. In February 2002, they began operating a small two-room clinic, initially focused on providing Ob/Gyn and family practice-based care to immigrant and refugee families. Over time, AHSFHC has experienced significant growth and transitioned into a full-time Federally Qualified Health Center (FQHC) in the West Ridge neighborhood on March 1, 2004. They are proud to be recognized as a Patient-Centered Medical Home (PCMH).

Alivio Medical Center

Alivio Medical Center opened its doors to an eagerly awaiting community on January 4, 1989. On that day, the vision to create a community-based health center, responsive and culturally sensitive to many of Chicago’s most underserved and vulnerable communities, was finally realized. The Chicago neighborhoods of Pilsen, Little Village and Back of the Yards have been significant points of entry for immigrants and remain the home to a growing Mexican immigrant population in addition to an expanding community of Mexican Americans.

Today, Alivio Medical Center serves families and individuals who historically have had limited access to healthcare for many reasons including: language and cultural barriers, intimidation of traditional healthcare institutions, limited awareness of available resources and/or fear of seeking help due to immigration status. Consequently, health promotion, education and prevention are integral parts of our comprehensive programs. Alivio’s bilingual, bicultural providers serve all patients regardless of ability to pay.

Community Health Care, Inc.

Community Health Care, Inc. (CHC) offers primary, preventive, acute and chronic care, as well as dental, behavioral health, OB/GYN, express care, lab services, pharmacy, school-based health, outreach care, mobile health service, WIC, nutrition, and health education services. Without CHC, many patients would be forced to rely on emergency rooms for their primary care, or they would go without care altogether. In recent years, CHC has made great strides, many of which were made possible with the assistance of generous donors in the Quad Cities area. The number of patients that CHC serves on an annual basis has grown to nearly 50,000 individuals, more than 12 percent of the Quad City’s total population.

Erie Family Health Centers

Motivated by the belief that healthcare is a human right, they provide high quality affordable care to support healthier people, families, and communities. Erie delivers holistic care to help every member of the family stay healthy and active from infancy through adulthood.

Esperanza Health Centers

In 2004, residents of the neighborhoods of Pilsen and Little Village on Chicago’s Southwest Side joined administrators from nearby Saint Anthony Hospital to address the critical lack of accessible health services in the area. They opened a community health center on South California Avenue called Esperanza, the Spanish word for “hope.”

Since then, Esperanza has provided bilingual, high quality primary care, behavioral health and wellness services to the community, regardless of immigration status, insurance status, or ability to pay. Their five sites in Brighton Park, Little Village, Marquette Park, South Lawndale and West Lawn deliver care to more than 50,000 patients each year from communities across the Southwest Side.

Their commitment to offering care of the highest quality is made clear by our designation as a National Quality Leader and Health Center Quality Leader by HRSA consecutively for the past five years, ranking Esperanza among the top health centers in the nation for our quality of care.

Greater Family Health

Greater Family Health believes that everyone deserves access to high quality, affordable health care delivered with respect by compassionate and qualified professionals. They have a commitment to improve the quality of care we provide and the health of their patients. This commitment led Greater Family Health to pursue accreditation by the Joint Commission and the certification as a Primary Care Medical Home. Because of our Primary Care Medical Home status, their patients benefit from having access to an interdisciplinary team of medical, dental, and behavioral health providers, all working in partnership with the patient to improve their quality of life and their families.

Greater Family Health works to eliminate any barriers that may keep their patients from accessing services. They do this by providing 24-hour access to providers, comprehensive care management, transportation assistance, access to reduced-price pharmaceuticals, on-site laboratories, same and next-day appointments, bilingual staff and translation services.
