Patient Centered Medical Home

Patient-centered medical home or primary care medical home (PCMH) is a model of health care that puts the patient at the forefront of their care. PCMH recognized practices aim to improve quality, efficiency, and effectiveness of care that is based on the patient’s preferences and needs. AHRQ defines the medical home with five distinctive features: comprehensive care, patient-centered, coordinated care, accessible services, and quality and safety.

Patient-Centered Medical Home office hours are provided by IPHCA staff through ongoing webinars for health centers getting started with the recognition process, transitioning, or renewing. If you would like to request one-on-one technical assistance, please contact Naila Quraishi at nquraishi@iphca.org.
