Are you eligible for Organizational Membership?

To join, your organization must be designated as a Federally Qualified Health Center, receiving section 330 Federal funding OR be designated as an FQHC “look-alike.”

To join, your organization should be in the process of applying for Federally Qualified Health Center or FQHC Look-A-Like status.

Associate membership is available to any organization, agency, foundation or association that supports IPHCA’s purpose and goals and is not otherwise eligible for Organizational or Network membership.

Are you eligible for Business Membership?

To join, your organization must do business with any Federally Qualified Health Center or FQHC “look-alike” that supports the goals about IPHCA. Organizations that wish to join the Primary Health Purchasing Alliance (PHPA) must be a Business Member.

Are you eligible for Network Membership?

To join, you must be part of an integrated service network that receives or has received funding from the federal Health Resources and Services Administration Integrated Services Development Initiative (ISDI) or Shared Integrated Management Information Systems (SIMIS) programs.

IPHCA Member Esperanza Health Centers Honored with Four Awards for Quality, Outcomes

Founded in 2004, Esperanza provides high-quality, bilingual primary care, behavioral health and wellness services across four sites on Chicago’s Southwest Side. In 2019, Esperanza received four awards totaling over $190k from the Health Resources Services Administration—funds earmarked for health centers that achieved the best overall clinical outcomes and demonstrated consistently high-quality care. At $97k, the largest award recognized Esperanza’s efforts to promote behavioral health, diabetes, and cardiovascular care in 2018. Esperanza ranked in the top 1-2% of all health centers in measures of quality related to the care of these health issues.
