Patient Centered Medical Home

The process of Patient Centered Medical Home (PCMH) recognition enables community health centers (CHCs) to work toward improving quality of care and outcomes for their populations, increasing access and providing care in a cost effective manner.

QI Interest Group: 

The PCMH Interest Group meets via conference call to discuss and learn about topics related to quality improvement, practice transformation and medical home recognition. This group is targeted to health center staff working on quality improvement initiatives. This is a learning community for health centers to learn best practices from each other and share their experiences.

Topics include:

  • Updates on HRSA sponsored initiatives
  • National Center for Quality Assurance (NCQA) PCMH recognition program (PCMH 2011 vs. PCMH 2014)
  • PCMH accreditation programs through The Joint Commission (TJC) or the Accreditation Association for Ambulatory Health Care (AAAHC)
  • Ideas for enhanced access and continuity (open access scheduling, patient portals)
  • Incorporating PCMH into a health center’s Quality Improvement Plan
  • Strategies for building support from leadership (board of directors, senior leaders)
  • Sample timelines for the PCMH recognition process
  • Resources for PCMH self-assessment 
  • Tracking referrals and follow-up care
  • Proactively engaging high risk patients
  • Providing resources for self-management support
  • Building provider panels
  • Team-based care
