The process of Patient Centered Medical Home (PCMH) recognition enables community health centers (CHCs) to work toward improving quality of care and outcomes for their populations, increasing access and providing care in a cost effective manner.
Patient-Centered Medical Home Recognition Through the National Committee on Quality Assurance (July 8, 2011)
The Health Resources and Services Administration (HRSA) is currently offering the Patient-Centered Medical Health Home Initiative (PCMHHI), which will cover the cost of recognition through the NCQA. This article provides an overview of the process for health centers participating in the initiative. See below as well for additional resources for pursuing NCQA PCMH recognition.

IPHCA Supports CHC Efforts to Pursue PCMH Recognition (June 1, 2011)
On April 20, 2011, IPHCA hosted "Patient-Centered Medical Home: Road to Recognition," a one-day training for member health centers to provide the most current information and tools related to PCMH recognition.
 
This article answers the following questions: What is a Patient-Centered Medical Home? Is the U.S. Health Resources and Services Administration (HRSA) supporting community health centers (CHCs) in attaining PCMH recognition? How are health centers supposed to rally already stretched resources around another quality improvement initiative? Will PCMH recognition really make a difference for patients and organization?
 
PCMH Interest Group:
The PCMH Interest Group meets bi-monthly via conference call to discuss and learn about topics related to practice transformation and medical home recognition.  This group is targeted toward health center staff who are working on operations or quality improvement initiatives related to patient-centered medical home.  Participants share their experiences with practice transformation as a part of this learning community. 
 
Topics include:
  • Updates on HRSA sponsored PCMH initiatives
  • National Center for Quality Assurance (NCQA) PCMH recognition program
  • PCMH accreditation programs through The Joint Commission (TJC) or the Accreditation Association for Ambulatory Health Care (AAAHC)
  • Ideas for enhanced access and continuity (open access scheduling, patient portals)
  • Incorporating PCMH into a health center’s Quality Improvement Plan
  • Strategies for building support from leadership (board of directors, senior leaders)
  • Sample timelines for the PCMH recognition process
  • Results from PCMH Demonstration projects
  • Resources for PCMH self-assessment 
  •  Tracking referrals and follow up care
  • Proactively engaging high risk patients
  • Providing resources for self-management support
  • Building provider panels
  •  Engaging the whole team in caring for patients
 
Pursuing NCQA PCMH Recognition
A health center can become recognized as a PCMH through the National Committee for Quality Assurance (NCQA). NCQA and the Health Services Resource Administration (HRSA) have partnered to support and encourage CHCs to achieve recognition.
