Training Overview

This program is an in-depth guide to NCQA recognition requirements and the process for achieving PCMH recognition.

What You Will Learn

At the conclusion of the program, participants will have the knowledge they need to:

  • Identify the core attributes of patient-centered medical homes.
  • Identify required documentation and determine how to present it.
  • Describe processes and procedures that demonstrate transformation into the medical home model.
  • Work with an assigned NCQA Representative on a plan to earn recognition.
  • Examine the more challenging aspects of the requirements in a variety of practice environments.

Training Overview

This advanced course is intended for individuals who have applied NCQA PCMH recognition requirements in the practice setting.

What You Will Learn

At the conclusion of the program, participants will have the knowledge they need to:

  • Discuss vendor prevalidation, autocredit, electronic clinical quality measurement, and certified electronic reporting to facilitate recognition and annual reporting.
  • Explain how elective criteria build upon core criteria within a competency.
  • List the opportunities and challenges to succeed in the PCMH model of care.
  • Determine a strategy for preparing a practice for the three (3) virtual check-ins and begin to create a transformation plan.
  • Describe the types of evidence a practice can provide to meet the intent of a criterion, competency, and concept.
  • Explain the role of PCMH Certified Content Experts in assisting practices through the Commit, Transform, and Succeed parts of the PCMH recognition.

Patient-centered medical home or primary care medical home (PCMH) is a model of health care that puts the patient at the forefront of their care. PCMH recognized practices aim to improve quality, efficiency, and effectiveness of care that is based on the patient’s preferences and needs. AHRQ defines the medical home with five distinctive features: comprehensive care, patient-centered, coordinated care, accessible services, and quality and safety.

 

Patient-Centered Medical Home office hours are provided by IPHCA staff through ongoing webinars for health centers getting started with the recognition process, transitioning, or renewing.
